Incident Reports

During the rehearsal process, the PSM will ensure that there is a Safety Representative identified (this may be the PSM themselves). This individual will be responsible for ensuring that an Incident Report (located in Annex B of this Handbook) is completed including the name of anyone involved in the incident and any actions taken as a result of the incident. PSMs should turn the report in to the Producer or slide it in the blue box located in the Green Room. PSMs should also keep a copy in their book.
